SDOH Month: MiHIN Advancing Health Equity Efforts in Michigan

With January 2024 being Social Determinants of Health (SDOH) Month in Michigan, the state is already advancing health equity efforts through the good work done by Michigan Health Information Network (MiHIN).

Lisa Nicolaou, Program Director, Cross-Sector Data Sharing at MiHIN, recently authored tthis Op-Ed in Bridge Michigan about how her organization is leading the way by fostering collaboration among historically siloed healthcare and social service providers.

The article discusses how MiHIN is harnessing the combined power of health and social care data – developing “a holistic understanding of an individual’s needs, optimizing resource allocation and developing targeted interventions that improve care outcomes for Michiganders.”

Here are other ways that health equity efforts are advancing in Michigan:

  • MiHIN facilitated a trailblazing “Interoperability Pledge.” This effort brought together several of the most prominent software companies to better eliminate information silos and improve care coordination.

  • Michigan’s newly established Community Information Exchange (CIE) Advisory Committee. This taskforce maximizes a person-centered approach by ensuring social care information is collected only as needed and is stored safely.

In addition, MiHIN announced last year that it is advancing bidirectional data exchange for the Nottawaseppi Huron Band of the Potawatomi (NHBP).

By joining MiHIN, the NHBP will become the first federally recognized tribe in the state of Michigan to initiate its own access to the Active Care Relationship Service (ACRS), a centralized solution for sharing patient health information across different healthcare providers and organizations.

Advancing SDOH is a critical priority for many states, and Michigan is laying the foundation for other regions to effectively break down the barriers of health data exchange to ensure all citizens have the right access to care.
