Los Angeles Network for Enhanced Services (LANES): Connecting Beyond the EHR

As a community driven Health Information Exchange (HIE) network, Los Angeles Network for Enhanced Services (LANES) enables comprehensive health data, clinical data insights and population health capabilities at the point of care throughout L.A. County’s healthcare ecosystem.

LANES was originally created with the purpose to improve community health with a special focus on the underserved population of Los Angeles – the most populous U.S. county with more than 10.4 million residents representing 140+ cultures and considerable income variation.

In 2020, LANES CEO Ali Modaressi authored a Healthcare IT News guest article that highlighted the core mission behind the HIE, which is to “connect beyond the EHR.” His vision is to aggregate medical records for enhancing care quality for LA-based providers and the overall patient community.

To meet this need, LANES connects local providers across the care continuum to the most up-to-date patient data when needed from various sources. In addition, LANES’ clinical and operational workflows generate cross-community data insights to deliver the most appropriate treatments in real-time, eliminate wasteful or redundant procedures, and identify and proactively manage risk.

The organization achieves this through its population health solutions that:

  • Aggregate and synthesize cohorts of patient information in one convenient place focused on key health conditions to highlight patients requiring emergent attention.

  • Identify and intervene in cases of high-risk, rising risk and high utilization of healthcare resources ─ empowering providers to focus their time and attention on patients who are most in need.

  • Connect to L.A. County Department of Health Services (DHS) empaneled life management (ELM) registry-based performance program, Department of Public Health’s Precision Medicine Program and Department of Mental Health to support local public health needs.

  • Integrate social determinants of health (SDoH) data so providers can leverage SDoH information with confidence in developing treatment plans for populations encumbered with health and social inequities including those with lingering COVID-19 ailments.
